All workplace injuries must be reported to a Patriot HR Solutions representative immediately – regardless of how insignificant the injury or complaint may be at the time of the incident.
The injury reporting form (first report of injury) requires immediate completion and must be emailed promptly to firstname.lastname@example.org.
Please send any employee requiring work-related medical attention to a provider with the authorization for treatment form. You may also print the first fill form and provide it to any injured employee. This is used for the employee’s first prescription. Once the claim is set up, the employee will be provided with another prescription card by mail to be used after the first fill.
A Patriot HR Solutions representative will help to coordinate medical triage, facilitate the filing of the claim, and manage the ongoing claims administration efforts including return-to-work expectations.

If an employee declines medical treatment, please have them complete and sign the refusal of medical treatment form. Always keep a copy on file and forward to email@example.com.
